Memory Assessment Nurse Specialist

We are seeking a highly skilled and experienced Memory Assessment Nurse to join our team at Nottinghamshire Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for assessing and diagnosing individuals with cognitive impairment, providing person-centered care, and collaborating with colleagues to reduce waiting times.
Key Responsibilities- Screen referrals for appropriateness and undertake initial assessments
- Create treatment plans and liaise with the multidisciplinary team to explore symptoms and causes
- Refer for additional assessments and signpost for post-diagnostic support
We are looking for a candidate with a strong background in mental health, particularly in older adults. Essential qualifications include a RNMH qualification, and desirable qualifications include a non-medical prescribing qualification and clinical supervision course.
Our ideal candidate will have substantial experience of multidisciplinary team working, CPD relevant to the current post, and current knowledge and understanding of older people's mental health needs. They will also have the ability to demonstrate clinical competence and evidence knowledge of dementia.
