Primary Responsibilities
Demonstrating clinical expertise in both Older People’s Mental Health Services (OPMHS) and adult care is required.
Working collaboratively as part of a Multi-Disciplinary Team, reporting findings to colleagues both verbally and in writing, while seeking guidance and support when needed.
Each patient assigned as a Lead Professional will receive a personalized care plan with review dates, adhering to Community Mental Health Nurse and Care Programme Approach standards.
The role involves actively contributing to the establishment of care standards and implementing quality improvement initiatives.
Promoting adherence to prescribed medications and treatments, while noting and reporting any non-compliance or side effects, is essential.
Building and maintaining strong partnerships with all stakeholders, including statutory, third sector, and voluntary organizations, is a key responsibility.
Advising patients, caregivers, and significant others about available resources and facilitating referrals to appropriate agencies is necessary.
Understanding the requirements of the Mental Health Act 1983, Mental Capacity Act 2005, Care Act 2014, Care Programme Approach, and Deprivation of Liberty safeguards as they pertain to psychiatric liaison services is crucial.
Adhering to nursing policies and procedures regarding medication administration and depot injections is required.
