We are seeking a highly skilled and compassionate Care Manager to join our team at Hallandhouse. As a key member of our care team, you will be responsible for providing exceptional support to our residents with learning disabilities, complex behaviors, and physical/sensory disabilities.
Key Responsibilities- Coordinate the effective planning of shifts, ensuring that staff members' skills and experience match the needs of the residents.
- Liaise with external professionals to ensure that staff members understand and implement guidelines accordingly.
- Administer medications safely, ensuring that any changes are communicated to staff members in a timely manner.
- Monitor the wellbeing of residents, checking daily records and healthcare appointment records.
- Coordinate admissions and discharges from the hospital, ensuring that staff allocations are done to maintain consistency of care for the residents.
- Act in the best interest of the residents, supporting them with Deprivation of Liberty Safeguards (DOLS) appointments.
- Review and update care plans when changes occur and individual needs are changed.
- Support staff and residents with healthy eating initiatives, coordinating Apitito menus.
- Organise and coordinate residents' house meetings, involving residents in the house community decision-making process.
- Monitor the safety of residents, recording and reporting incidents and accidents to the Registered Manager/Deputy Manager.
- Implement Positive Behaviour Support plans, updating proactive strategies, and working with the team to create a learning environment for the residents.
- Promote 'Dignity and Respect' amongst the residents and the staff in the service.
- Monitor that the privacy of the residents is maintained at all times.
- Review and plan staff rotas, filling gaps with staff members, bank staff, or authorised agency personnel as directed by the registered manager.
- Develop warm and trusting relationships with residents to encourage them to express their needs, views, and concerns.
- Monitor staff absences and lateness, addressing any issues with frequent lateness or absences.
- Organise staff house meetings and plan house events, such as birthdays and anniversaries.
- Delegate tasks to staff members, ensuring they have the necessary skills and knowledge to complete them.
- Evaluate staff competencies by conducting assessments around safe medication administration, mental capacity, safeguarding adults at risk, infection control, health and safety, person-centred care, privacy, and dignity.
- Monitor daily paperwork completion, including ABC charts, incident forms, care records, health appointment forms, and seizure charts.
- Conduct spot checks and daily walk-arounds with night staff.
- Ensure that all assistive technology is working and report any issues to the management team or maintenance.
- Conduct relevant audits to maintain the safety and security of residents and the service.
- Report health and safety issues to management and follow up with the maintenance team.
- Maintain constant communication with residents' families and friends, informing and updating them about changes in the individual's care.
- Conduct weekly fire drills and maintain accurate records.
- Attend weekly management meetings and provide information and updates on changes in care, behaviour, and health concerns.
- Maintain a positive culture in the home by encouraging engagement in internal and external activities.
- Monitor infection control procedures in the home, ensuring medication trolleys are clean and tidy and vehicles are free from debris.
- Participate in a 24-hour rotational shift pattern that includes regular weekend and bank holiday working.
- Lead and motivate staff to perform their best by providing coaching and daily supervision.
- Conduct monthly supervision meetings, ensuring staff members are prepared for the meeting, and encourage staff participation and ownership of delegated tasks.
- Act as a role model at all times, and address inappropriate behaviour, poor conduct, and negative attitudes towards staff or residents in the service.
- Provide constructive feedback to staff and set achievable goals while ensuring staff understand what is expected of them.
- Support staff in implementing resident behaviour and health guidelines, and help staff meet their training requirements by completing online training.
- Contribute to service users' reviews by providing verbal and written reports and attending Support Planning meetings.
- Maintain a professional code of confidentiality and work as part of the team to achieve goals set out in the individual care plan.
- Ensure work practices comply with the home's policy on Equality and Diversity at all times.
- Support residents in maintaining safety, security, and comfort in the home.
- Use appropriate strategies, management guidelines, and interventions specified by Person-Centred Plans to support individuals who express their frustrations and needs through challenging behaviour.
- Support residents in developing a socially valued lifestyle, including culturally and age-appropriate experiences that build on their strengths, interests, and aspirations.
- Promote the self-esteem, happiness, and emotional health of service users.
- Respect the right to privacy of service users and ensure their dignity is maintained.
- Implement positive risk management strategies in line with Person-Centred Plans.
- Observe and monitor the emotional and physical well-being of service users and inform relevant staff and involved professionals of any concerns or significant changes in needs, behaviour, and circumstances.
- Use appropriate communication techniques based on the needs of the service user.
- Conduct and record financial transactions involving service users in accordance with company guidelines.
- Drive service users to trips and community facilities in the company vehicle, if requested and after passing appropriate reviews.
- Regularly attend staff meetings to stay informed and contribute to discussions.
- Support the Registered Manager in carrying out audits, spot checks, and quality assurance surveys to maintain high standards of care.
- Attend Multi-disciplinary meetings as required to ensure effective communication and collaboration between professionals.
- Conduct thorough inductions for new staff members, including familiarising them with care standards, the Code of Conduct in health and social care, and other relevant policies and procedures.
- Support staff in completing their 'Care Certificates' and evaluate their performance to identify areas for improvement.
- Adhere to management protocols and guidelines for individuals with learning disabilities to ensure consistency in care.
- Prioritise the physical and mental wellbeing of service users, ensuring they receive regular health checks and promoting healthy lifestyle habits.
- Contribute constructively to Individual Performance Reviews as part of the team effort to achieve the goals set out in the individual care plan.
