Hospital at Home Frailty Nurse Specialist

We are seeking a skilled and compassionate nurse to join our team as a Hospital at Home Frailty Nurse. This is an exciting opportunity to work with a dedicated team to provide high-quality care to patients with frailty in their own homes.
About the RoleAs a Hospital at Home Frailty Nurse, you will be responsible for providing comprehensive assessment and care to patients with frailty in their own homes. You will work closely with the multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
You will have the opportunity to work with a range of patients, including those with complex medical needs, and will be expected to provide high-quality care that is tailored to each patient's specific requirements.
Key Responsibilities- Provide comprehensive assessment and care to patients with frailty in their own homes
- Develop and implement individualized care plans that meet the unique needs of each patient
- Work closely with the multidisciplinary team to ensure seamless care coordination
- Provide education and support to patients and their families to promote independence and self-management
- Participate in quality improvement initiatives to enhance patient outcomes and experience
To be successful in this role, you will need to have:
- Registered Nurse qualification and current registration with the NMC
- Experience of working with patients with frailty and complex medical needs
- Strong assessment and care planning skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
