Position Overview
We are seeking meticulous individuals to become part of our Referral Coordination Team. You will play a crucial role in fostering effective communication and information exchange, ensuring that patients are kept informed of their referral outcomes as directed by our clinical professionals.
Weekly Commitment - hours
Key Responsibilities
- Ensure timely processing of incoming referrals in accordance with established training and within specified timeframes.
- Provide assistance to patients, healthcare providers, and community health organizations regarding referral processes.
- Develop and apply a comprehensive understanding of referral pathways.
- Manage requests for medical reports and/or copies of patient records.
- Guarantee that referrals are handled promptly and in alignment with designated pathways, organizational policies, and procedures.

Our facilities are conveniently located and well-connected by public transport, allowing us to maintain a unified patient list while ensuring that patients receive care from dedicated multidisciplinary teams, including GPs, Nurses, Physician Associates, Paramedics, Advanced Nurse Practitioners, and Pharmacists. These teams are supported by administrative staff and central management functions, including HR, IT, and Clinical Administration.
Each facility operates with a Site Manager and GP partners, actively participating in training programs for both GPs and allied health professionals. Nexus collaborates closely with local healthcare networks, with partners serving as Clinical Directors.

Qualifications
Essential
- A minimum of 3 GCSEs at grades A-C (or equivalent), including English Language, or relevant experience.
- Experience in a healthcare environment.
Desirable
- Proficiency in IT.
- Familiarity with Emis software.
- Training in medical terminology.
- Understanding of NHS operations and primary care.
- Experience in summarizing medical records.
