About the Role
We are seeking a highly motivated and enthusiastic Social Worker to join our Single Point of Access Team at North Somerset Council. As a key member of our team, you will be responsible for providing timely advice, information, and triage of referrals received to adult care and community health services.

Key Responsibilities
- Work within a diverse team of Health and Social Care professionals to provide high-quality services to our residents.
- Develop and maintain strong relationships with colleagues and stakeholders to ensure effective communication and collaboration.
- Conduct thorough assessments and risk assessments to identify the needs of our residents and develop effective care plans.
- Work closely with our community-based care management clinics to ensure seamless delivery of services.

Requirements
To be successful in this role, you will need:
- A degree in Social Work or a related field.
- Registration with the Health and Care Professions Council (HCPC).
- Excellent communication and interpersonal skills.
- Able to work effectively in a fast-paced environment.
