About the Role:
We are seeking a Support Worker to join our team at Case Management Cymru Ltd. As a Support Worker, you will provide support and care to individuals in the community, promoting their independence and well-being.
Key Responsibilities:
- Provide support and care to individuals in the community, promoting their independence and well-being.
- Work collaboratively with colleagues and other agencies to ensure the best possible outcomes for service users.
- Develop and maintain relationships with service users, their families, and other stakeholders.
- Contribute to the development and implementation of individual support plans.
- Monitor and report on service user progress and outcomes.
Requirements:
- Experience of working in a support or care role.
- Ability to work collaboratively with colleagues and other agencies.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
- Flexibility to work a variety of shifts, including evenings and weekends.
