Senior Reablement Worker

We are seeking a highly skilled and dedicated individual to join our Reablement team as a Senior Reablement Worker. As a key member of our team, you will play a vital role in supporting individuals to regain their independence and confidence in their daily lives.

The successful candidate will have experience in a care setting, with a strong focus on delivering high-quality care and support to individuals in their own homes. You will work closely with our integrated team to develop and implement personalized care plans, ensuring that each individual receives the support they need to achieve their goals.

• Support and supervise the Reablement care at home team on a day-to-day basis
• Develop and implement personalized care plans to maximize independence and support individuals to remain at home
• Work effectively with the integrated team to support individuals to develop or regain skills, confidence, and community connections
• Provide excellent communication and interpersonal skills, both verbally and in writing
