We are seeking a passionate and experienced Team Manager to lead our Integrated Hospital Discharge Social Work Team. This is an exciting opportunity to make a real difference in the lives of adults within Salford.
As a key member of our Adult Social Care team, you will play a vital role in ensuring that high-quality, integrated care services are delivered to adults across Salford. You will be responsible for managing and motivating a team of dedicated social workers, promoting effective collaboration with healthcare professionals, and driving continuous improvement within the service.
Key Responsibilities
- Lead and manage a team of social workers, providing supervision, support, and guidance to ensure high-quality care delivery.
- Develop and implement strategies to enhance the effectiveness of hospital discharge processes, promoting seamless transitions for patients.
- Collaborate closely with healthcare professionals, local authorities, and other stakeholders to coordinate comprehensive care plans for adults.
- Ensure compliance with relevant legislation, policies, and best practices within the social work field.
