Job Summary

We are seeking a highly skilled and dedicated Community Staff Nurse to join our team at Wirral Community Health and Care NHS Foundation Trust. As a Community Staff Nurse, you will play a vital role in providing high-quality care to patients in their own homes, promoting independence and improving health outcomes.

Main Responsibilities

As a Community Staff Nurse, your key responsibilities will include:

  • Providing comprehensive nursing care to patients in their own homes, including assessments, planning, implementation, and evaluation of care.
  • Working collaboratively with other healthcare professionals to ensure seamless care delivery and effective communication.
  • Developing and maintaining strong relationships with patients, families, and carers to promote trust and confidence in the care provided.
  • Participating in clinical governance and quality improvement initiatives to ensure the highest standards of care are maintained.
  • Staying up-to-date with the latest clinical guidelines, policies, and procedures to ensure evidence-based practice.

Requirements

To be successful in this role, you will need:

  • A registered General Nurse qualification and current NMC registration.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
  • A sound knowledge of recent changes within the NHS and current professional issues.
  • The ability to work independently and as part of a team, with a flexible approach to working hours and shifts.
  • Access to a car for work purposes.
