Job Description

About the Role

We are seeking a highly motivated and compassionate Care Support Specialist to join our team at Torfaen County Borough Council. As a Care Support Specialist, you will play a vital role in providing high-quality care and support to individuals in their own homes, enabling them to live independently and safely.

Key Responsibilities

  • Deliver person-centered care and support to service users, promoting their independence and well-being.
  • Work collaboratively with colleagues and other agencies to ensure seamless care and support.
  • Conduct thorough assessments and develop personalized care plans to meet the unique needs of each service user.
  • Provide emotional support and companionship to service users, helping to alleviate feelings of loneliness and isolation.
  • Maintain accurate and up-to-date records of service user interactions and care plans.

Requirements

  • Basic understanding of care in the community and experience of caring for people in a paid or voluntary capacity.
  • Ability to work independently and as part of a team, with a flexible approach to meet the changing needs of service users.
  • Current driving license and use of transport is essential.
  • Qualification to NVQ level II in Care or willingness to work towards Level 2 Diploma in Health & Social Care (QCF).
  • Willingness to register with Social Care Wales.
