Job Title: Occupational Therapist - Reablement Specialist

**Job Summary:**

We are seeking two fully qualified Occupational Therapists to join our team at Tripod Partners. As a Reablement Specialist, you will be responsible for conducting face-to-face reviews and assessments for adults of all ages in relation to reablement, accommodation, equipment, and adaptations.

Key Responsibilities:

  • Conduct face-to-face reviews and assessments for adults of all ages
  • Undertake reablement, accommodation, equipment, and adaptations assessments
  • Triage to establish if community resources can be used instead of paid statutory services
  • Work with clients to develop personalized plans to achieve their goals

Requirements:

  • Full qualification as an Occupational Therapist
  • Experience in reablement, community care, and assessment and triage
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
