Sanctuary Personnel Ltd, a leading recruitment agency, has a new permanent position available for a Social Worker within the Adult Learning Disability Team to work Full Time.
The salary for this permanent Social Worker job is up to £46,578 per annum.
- Ensure the delivery of high-quality care and support services that promote the wellbeing of vulnerable adults and their carers.
- Participate in the operation of duty and respond accordingly to need, which will involve Safeguarding enquiries and Best Interest Assessment (BIA) when required.
- Provide an assessment service to individuals, their carers and other agencies in accordance with Care Act 2014 criteria.
- Initiate and co-ordinate care and support plans for and with individual service users and support plans for carers, in conjunction with professional colleagues and representatives of voluntary and private agencies.
- Monitor the efficiency and effectiveness of individual care and support plans.
- Review the needs of individual service users and carers, and maintain client records in accordance with Council Policy.
- Respond to and where possible resolve complaints within the Council's complaints procedure.
- Respond to new and changed legislation in respect of Adult Services and implement as directed.
- Ensure effective use of all available resources including voluntary sector to meet the eligible needs of the adult.
- Co-ordinate, participate and where appropriate chair case conferences and planning meetings including Safeguarding meetings.
- Develop and maintain strong relationships with service users, carers, and other stakeholders to ensure effective delivery of services.
- Contribute to the development and implementation of policies and procedures that promote the wellbeing of vulnerable adults and their carers.
