Giving Home Health Care is seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Nurse Care Specialist. This is a 1099 contract opportunity, where you will provide patient care in the comfort of their own residence.
Key Responsibilities- Develop and Implement Care Plans: Execute physician-prescribed plans of care, ensuring the highest level of patient satisfaction and well-being.
- Compliant Documentation: Complete accurate and timely documentation of care provided in our system of record.
- Medication Management: Manage and administer prescribed medication, treatment, and therapies as ordered by the patient's physician.
- Patient Assessments: Conduct thorough patient assessments, coordination of care, and monitoring of vital signs and GI intake/output.
- Wound Care and Skin Integrity: Assess skin integrity and administer wound care as needed.
- Communication and Education: Communicate effectively with family members, physicians, case managers, and other prescribed individuals regarding the patient's health. Provide education and training to patients, their families, and/or Home Health Aides.
- Team Collaboration: Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
- Additional Services: Perform other personal care services as necessary to meet the patient's needs.
- Education and Licensure: Graduate of an accredited college or university, Associate or Baccalaureate School of Nursing. Current RN or LPN licensure.
- Manual Dexterity and Coordination: Possess hand-eye coordination and manual dexterity to perform various tasks.
- Medical Management: Ability to provide basic medical management up to medically fragile end-of-life care.
- Experience: 1+ years of experience as an RN/LPN in a clinical or home health setting.
Compensation: $35 - $50 an hour (RN: up to $50 an hour, LPN: up to $35 an hour). Part-time schedule: 2-4 hour visits per month. Flexible schedule to be worked out with the patient.
