About the Role

We are seeking a highly skilled and experienced administrator to join our Exeter multi-disciplinary team as a Health and Social Care Coordinator. This is a challenging and rewarding role that requires excellent communication, organisation, and prioritisation skills.

Key Responsibilities
  • Provide administrative support to the local Health and Social Care Team, including coordination of contact and outcome information.
  • Facilitate the 'core group' function, including preparation for core group discussions, checking patient history, and involvement with other agencies.
  • Coordinate the provision of timely and effective support to clients, families, and clinicians, working closely with other parts of the wider community teams.
  • Identify urgent assessments that need action, alert and liaise with local Social Care Team Managers / Community Nurse, Urgent Community Response and Therapy Managers.
  • Provide feedback to referrers about the outcomes of contacts and assessments in accordance with the agreed quality standards.
  • Maintain IT based information systems and responsibility for key performance data.
  • Ensure the IM&T requirements for recording team activity are adhered to in collaboration with other team members.
  • Provide key performance/activity data within the local Exeter Community teams.
  • Work as a generic coordinator supporting Exeter Community, to include Nursing, Therapy, Business Support and Urgent Community Response.
  • Be trained and in the use of EPIC and CF6 along with other relevant IT programmes.
  • Record, send and store information in line with the Data Protection Act and Caldicott principles.
  • Plan and respond to work load according to operational priorities.
  • Participate in the induction of new staff to the cluster team.
  • Take part in regular performance appraisal.
  • Undertake any training required in order to maintain competency including mandatory training.
  • Supervise staff.
Requirements

We are looking for an experienced administrator with excellent IT skills, strong communication, and interpersonal skills. You will need to be able to work on your own initiative, prioritise tasks, and manage a high-volume workload. You should be a good team player, supportive, flexible, and willing to offer help to your colleagues in other teams as necessary. You should be resilient and able to cope with a busy office and a large workload where change is constant.
