Key Responsibilities:
  • Deliver care to patients with long-term conditions, palliative care, and rehabilitation needs.
  • Work closely with the community nursing team to provide a seamless service to our patients.
  • Contribute to the delivery of interventions for patients with complex needs.
  • Develop and maintain therapeutic relationships with patients and carers.
  • Communicate effectively with patients, carers, and other healthcare professionals.
  • Participate in clinical supervision and contribute to clinical audit.
  • Undertake mandatory training and participate in a Personal Development Review process.
Requirements:
  • NVQ 2 in health and social care or equivalent.
  • Experience of working with the general public and in a healthcare setting.
  • Good communication skills and ability to use own initiative.
  • Able to kneel, bend, and stoop, and work in cramped environments.
  • Must have a full UK driving license and have access to a vehicle.
