Key Responsibilities:- Deliver care to patients with long-term conditions, palliative care, and rehabilitation needs.
- Work closely with the community nursing team to provide a seamless service to our patients.
- Contribute to the delivery of interventions for patients with complex needs.
- Develop and maintain therapeutic relationships with patients and carers.
- Communicate effectively with patients, carers, and other healthcare professionals.
- Participate in clinical supervision and contribute to clinical audit.
- Undertake mandatory training and participate in a Personal Development Review process.
- NVQ 2 in health and social care or equivalent.
- Experience of working with the general public and in a healthcare setting.
- Good communication skills and ability to use own initiative.
- Able to kneel, bend, and stoop, and work in cramped environments.
- Must have a full UK driving license and have access to a vehicle.
