About the Role

We are seeking an experienced Clinical Nurse Specialist to join our team at Walsall Healthcare NHS Trust. The successful candidate will work in partnership with GPs, consultants, and other healthcare professionals to provide high-quality care to patients with complex acute conditions.

Key Responsibilities
  • Assess the physical and social needs of patients with complex acute conditions and develop individualized care plans.
  • Work collaboratively with multidisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
  • Provide extended and supplementary prescribing services to patients, based on best available evidence.
  • Negotiate and agree with patients, carers, and other care professionals on individual roles and responsibilities.
Person Specification
  • First-level degree or equivalent in a relevant field.
  • Evidence of continuing professional development and a commitment to ongoing learning.
  • Experience of teaching and assessing, with a focus on community-based care.
  • Nurse Prescriber qualification and experience of case management.
  • At least two years' experience at Band 6 level within the community, with a strong understanding of team dynamics and leadership.
