As a Rehabilitation Specialist, you will play a crucial role in our Learning Disabilities and Autism (LDA) Team, collaborating with various stakeholders to ensure the best outcomes for those we serve. This position is tailored for an Experienced Rehabilitation Specialist with a passion for supporting individuals with learning disabilities and neurodiversity across different age groups, particularly young adults transitioning into independence.
Key Responsibilities
Your responsibilities will encompass:
- Facilitating a smooth transition for all eligible young individuals aged 14 to 18 into adulthood, ensuring that adults with learning disabilities and/or autism receive the necessary support in accordance with established criteria.
- Encouraging independence in young and older adults, tailoring support to achieve successful and realistic outcomes.
- Implementing an enablement approach, assisting individuals in maintaining their independence within their own homes.
- Collaborating with families to clarify the role and responsibilities of our services for eligible adults.
- Building strong partnerships with service providers, community organizations, educational institutions, and other relevant entities.
- Contributing to a high-performing, efficient, and supportive team that embodies the principles of teamwork and collaboration.
- Utilizing a strengths-based methodology and your specialized skills to empower individuals, promote well-being, and enhance daily living through various interventions, including equipment provision, adaptations, manual handling, goal setting, and close collaboration with care providers and health professionals.
A solid understanding of relevant legislation and its application, including safeguarding principles, is essential. The following qualifications are required:
- A degree in Occupational Therapy or a related field.
- Registration with the Health and Care Professions Council (HCPC).
