About the Role

We are seeking a highly skilled and dedicated Social Worker to join our Community Mental Health Team in Tameside. As a key member of our team, you will play a vital role in providing high-quality social care to individuals with moderate to severe mental health problems.

Key Responsibilities:

  • Conduct comprehensive assessments and develop personalized care plans to support service users.
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery.
  • Provide mental health support and guidance to service users and their families.
  • Participate in court work and other statutory responsibilities as required.
  • Develop and maintain strong relationships with community partners and stakeholders.

Requirements:

  • Qualified Social Worker with relevant experience in adult social care and mental health practice.
  • Current driving license or supported driver with use of vehicle.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
