Job Title: Band 5 Community Mental Health Practitioner

An exciting opportunity has arisen for a skilled mental health professional to join our team at Southern Health NHS Foundation Trust. As a Band 5 Community Mental Health Practitioner, you will play a vital role in providing high-quality care to individuals with mental health needs.

Main Responsibilities:
  • Provide care coordination for individuals with mental health conditions, working closely with multidisciplinary teams to develop and implement personalized care plans.
  • Deliver evidence-based clinical interventions, including risk assessments and the recovery model, to support individuals in achieving their goals and improving their mental health.
  • Collaborate with community partners, including social workers, occupational therapists, and mental health nurses, to ensure seamless care coordination and effective service delivery.
Person Specification:
  • Mental health qualification as a social worker, occupational therapist, or mental health nurse.
  • Development role considered for current Band 5 applicants.
