About the Role

We are seeking a compassionate and experienced Community Support Worker to join our dedicated charity, helping to transform the lives of vision-impaired ex-service people.

As a Community Support Worker, you will support our members in Nottinghamshire, South Yorkshire, Lincolnshire, and Derbyshire to achieve their goals, while maximising their independence and wellbeing.

You will carry out risk assessments and identify the needs of individuals and how to meet them, responding to queries and feedback to better co-ordinate services.

Providing a member-led service, you will collaboratively plan and oversee the delivery of specialist interventions, wellbeing programmes, rehabilitation events, and other activities, focusing on promoting members' independence and wellbeing.

Additionally, you will:

  • Act as the service co-ordinator and main point of contact for members
  • Demonstrate and issue equipment
  • Support the development and work of volunteers
  • Support partnerships for joint working, member support, and future programmes
  • Ensure compliance with safeguarding and risk management policies

About You

To be considered as a Community Support Worker, you will need:

  • Experience working with people with disabilities to maximise their independence and wellbeing
  • Experience and knowledge of completing comprehensive, holistic assessments
  • Experience delivering remote and face-to-face services
  • The ability to use assessment tools and action plans
  • The ability to identify safeguarding concerns and act in line with policies
  • A relevant Level 4 qualification or equivalent experience
