We are seeking a highly skilled and dedicated Physiotherapist to join our Recovery at Home Team at South Tyneside and Sunderland NHS Foundation Trust.

• Work across Urgent Community Response, Discharge to Assess, and short-term rehabilitation services.
• Provide care to individuals in their own homes to prevent hospital admission or readmission.
• Undertake comprehensive holistic assessments of patients, prioritizing clinical need and implementing evidence-based care plans.
• Collaborate with multi-professional teams to ensure efficient, effective, and evidence-based practice.
• Develop and maintain expertise in physiotherapy to support the team in meeting service requirements.

• Registered with the Health and Care Professions Council (HCPC).
• Degree in Physiotherapy or a related field.
• Post-qualification experience relevant to this role.
• Experience of working with individuals with acute or chronic long-term conditions.
• Experience of multi-disciplinary working.
