We are seeking a highly motivated and enthusiastic Stroke Reviewer to join our Community Stroke Rehabilitation Team at Guy's and St Thomas' NHS Foundation Trust. As a Stroke Reviewer, you will play a vital role in providing comprehensive stroke review assessments to service users approximately 6 months post-stroke, using a validated and standardized tool.
Key Responsibilities- Conduct comprehensive stroke review assessments to identify new or unmet needs relating to the service user's stroke and determine appropriate actions to address these needs.
- Provide stroke prevention education, advice, and guidance to improve self-management and encourage meaningful engagement within the local community.
- Liaise with service users' carers to explore any needs that they have which should be addressed and clinically reason to determine appropriate onward referrals/care pathways.
- Communicate sensitive and complex information regarding rehabilitation to service users, carers, family, and other health and social care professionals and organizations.
- Screen service users' or carers' psychological state using validated mood screens and offer advice for onward support where needed.
- Complete medication reconciliation to ensure service users are taking the recommended prescription as stated on their hospital discharge report.
- Review walking aids and equipment that have been prescribed previously by clinicians to ensure they continue to meet the patients' moving and handling needs.
- Write a report to the service users' GP highlighting any concerns identified and rationale for actions that need to be completed by the GP.
- Make referrals to other services within the community as per the service users' needs, using analytical and clinical reasoning skills.
- Participate in the triage of referrals, using appropriate rationale to prioritize accordingly.
- Effectively manage own varied caseload, using prioritization and time management skills in a variety of community settings.
- English Language and Maths GCSE grade C and above.
- Formal qualification in relevant subjects: NVQ Level 3 (equivalent to 2 A Levels) or NVQ Level 4 (equivalent to Higher Education Certificate/BTEC).
- Excellent computer skills - ECDL or equivalent experience, including familiarity with Microsoft Office Programs.
- Experience of working for at least 1 year with adults with disability and/or older people in healthcare or related settings.
- Experience of working as part of a multidisciplinary team with other health and care professionals.
- Experience of direct patient care in a health or social care setting.
- Relevant in-service training/education in a healthcare setting.
- Experience of working in a rehabilitation setting.
- Experience of working in a community setting.
- Experience of working with stroke survivors and people with communication difficulties.
- Ability to communicate and present information, orally and in writing, in a clear and logical manner.
- Excellent communication, analytical, and interpersonal skills.
- Ability to work autonomously, recognizing one's own limitations of knowledge and seeking clinical advice as required.
- The ability to recognize potential breakdown and conflict when it occurs and generate potential solutions, seeking advice where necessary.
- The ability to develop practice to meet individual patients' circumstances, including due regard for cultural and linguistic differences.
- Able to demonstrate an understanding of the implications of disability on people's lives.
- The ability to manage and prioritize own work.
- Ability to work collaboratively with clients, families, carers, colleagues, and external services.
- Ability to treat people with respect, irrespective of age, gender, culture, religion, and sexuality.
