Main Responsibilities
  • Manage medicines after discharge from hospital
  • Risk stratification: identify patients at high risk of harm from medicines

  • Repeat prescribing: manage repeat prescribing reauthorisation process by reviewing patient requests

  • Long-term condition clinics
  • Service development: develop and manage new services built around new medicines or NICE guidance
About Us

The Southend East Primary Care Network is a dynamic and forward-thinking organisation, located in Norton Place, Shoebury. We cover approximately 37,000 patients and offer a range of appointments from our wide range of staff, fully utilising the ARRS budget. Our team consists of pharmacists, pharmacy technicians, social prescribers, health trainers, paramedics, physiotherapists, and assistant practitioners.

Job Requirements
  • Mandatory registration with the General Pharmaceutical Council
  • Excellent interpersonal, influencing, and negotiating skills
  • Excellent communication skills - both written and verbal
  • Able to plan, manage, monitor, advise, and review general medicine optimisation issues in core areas for long-term conditions
  • Able to obtain and analyse complex technical information
  • Recognises priorities when problem-solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate
