Senior Clinical Pharmacist - Community Services

The Community Medicines Optimisation Services (CMOS) team is a collaborative group focused on ensuring that patients receive optimal support in managing their medications. Working closely with various healthcare professionals, the team plays a crucial role in delivering high-quality care to individuals in their homes.

Key Responsibilities

As a Senior Clinical Pharmacist, your primary responsibilities will include:

  • Leading and managing the medication optimisation initiatives for community services, including crisis response and discharge planning.
  • Supervising and mentoring pharmacists and pharmacy technicians within the team.
  • Collaborating with multidisciplinary teams to enhance patient care coordination.
  • Implementing strategies to reduce hospital readmissions and improve medication safety and adherence.
  • Providing expert clinical pharmacy knowledge and guidance to healthcare staff and partner organizations.
  • Acting as an Independent Prescriber within the community services framework.
  • Developing and enforcing medication management policies to ensure safe and effective use of medications.
  • Leading new service initiatives and medication optimisation projects.
  • Facilitating training sessions for healthcare staff on medication management.
  • Participating in flexible working arrangements as needed to support service delivery.

Qualifications and Experience

To be successful in this role, candidates should possess:

  • Registration with the General Pharmaceutical Council.
  • A Master's Degree in Pharmacy or equivalent qualifications.
  • Relevant clinical diploma or postgraduate qualification.
  • Significant experience in a clinical pharmacy setting, with a focus on patient care.
  • Strong leadership and communication skills.
  • A commitment to continuous professional development.
