About the Role:

Key Responsibilities:

  • Collaborate with healthcare teams to ensure seamless care coordination and communication.

  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

Requirements:

  • Current licensure as a Physical Therapist in the state of New Jersey.
  • Experience working with geriatric populations, preferably in home care or senior living settings.
  • Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with flexibility to adapt to changing priorities and schedules.
