About the Role

We are seeking a highly motivated and compassionate Rehabilitation Support Worker to join our team at The Disabilities Trust. As a key member of our team, you will play a vital role in providing personalized support to individuals with acquired brain injuries, helping them to regain their independence and live fulfilling lives.

Main Responsibilities
  • Conduct yourself in a professional manner at all times, acting as a role model to colleagues and service users, and contributing to the delivery of quality services.
  • Facilitate daily routines of service users, promoting a high standard of personal hygiene and well-being.
  • Participate in the development of service user support plans, risk assessments, and programs, and complete written records as necessary.
  • Implement individual support plans to meet the needs of service users, highlighting any issues or concerns to the Team Leader to ensure quality services are provided.
About You

We are looking for someone with genuine empathy for our residents, who possesses the positivity, patience, and resilience to support them through challenging times. You should enjoy working as part of a team, take pride in getting the job done well, and have excellent face-to-face communication skills and a good standard of literacy.
