Role Overview

We are seeking a dedicated and compassionate individual to join our Enablement Team as a Relief Enablement Support Worker. As a key member of our team, you will play a vital role in supporting individuals to achieve their goals and maximize their independence.

Key Responsibilities
  • Provide person-centered support to individuals, empowering them to make informed decisions and take control of their lives.
  • Work collaboratively with the Enablement Locality Organiser, Supervisors, Social Workers, Occupational Therapists, and multi-agency partners to deliver high-quality support.
  • Develop and maintain strong relationships with service users, their families, and carers, ensuring their needs are met and their voices are heard.
  • Contribute to the development and implementation of individualized support plans, ensuring they are tailored to meet the unique needs of each service user.
  • Participate in regular supervision and training to enhance your skills and knowledge, ensuring you remain up-to-date with best practices and policies.
