Key Responsibilities
- Provide person-centred care and support to individuals, promoting their independence and well-being.
- Assist with daily living tasks, such as personal care, meal preparation, and medication management.
- Support individuals with their goals and aspirations, encouraging them to participate in activities and engage with their community.
- Work collaboratively with the care team to ensure seamless care and support.
- Contribute to the development and implementation of individual care plans.

Requirements
