Job Summary:
We are looking for a highly motivated and organized Social Worker to work with our team in providing high-quality care to adults in need. The successful candidate will have a strong background in social work, excellent assessment skills, and the ability to work effectively in a fast-paced environment.
Key Responsibilities:
- Assess and develop care plans for adults in need
- Work collaboratively with healthcare professionals and other stakeholders to ensure seamless care delivery
- Provide support and guidance to individuals and their families
- Contribute to the development and implementation of care policies and procedures
Requirements:
- Qualification in social work (e.g. DipSW, MSc, CQSW, CSS)
- Registration with the HCPC as a social worker
- Substantial experience as a qualified social worker
- Knowledge of adult social care eligibility criteria
- Excellent assessment skills
- A thorough understanding of adult safeguarding legislation and have attended safeguarding training
- A full understanding of, and commitment to, embedding personalisation in all aspects of social care
- The ability to cope with a case workload efficiently and effectively
- Good evidence-based recording skills, as your evidence may be required to be used in court
- The skills of a good communicator with an ability to deal with conflict
- The ability to work on your own initiative, organise your own workload, and work under pressure in response to competing demands and changing priorities
- Experience of the supervisory role
- Demonstrable IT skills
