Hospital Discharge Social Worker

Mbi Staffing is seeking a skilled and experienced Social Worker to join our Hospital Discharge team in Tower Hamlets. As a Hospital Discharge Social Worker, you will play a vital role in ensuring the safe and timely discharge of citizens with care and support needs from acute hospital settings and non-acute NHS community settings.

Daily Duties
  • Liaise with a team of Social Workers, case coordinators, and health professionals to promote and maintain independence for vulnerable adults.
  • Assist and participate in the management of the duty service and carry out screening of assessment notices to determine the appropriate discharge pathway.
  • Manage complex discharges, including identifying and carrying out Mental Capacity Assessments and chairing best interest meetings.
  • Complete strength-based assessments to implement person-centered support plans.
  • Protect adults at risk of abuse in accordance with the Care Act 2014 and Making Safeguarding Personal.
  • Have knowledge of discharge to assess pathways and the CHC process.
  • Skilled report writing.
Requirements
  • Degree or equivalent in social work.
  • Social Work England Registration.
  • Significant senior-level frontline experience within adults' services.
