Advanced Pharmacist for Community Services

We are seeking an experienced Oncology/Haematology Pharmacist to join our team at a leading NHS Hospital in Central London. As a Clinical Oncology & Haematology Pharmacist, you will be responsible for contributing to the specialist multidisciplinary team that provides inpatient, day case, and outpatient chemotherapy for a wide range of oncology and haemato-oncology cancer sites.
Key Responsibilities:- Oversee the safe management of high-risk systemic anticancer treatments.
- Provide clinical pharmacy services to wards and departments.
- Be responsible for the clinical pharmacy service to specialist haematology ward patients.
- Attend and contribute to the haematology ward round, and MDT meetings.
- Contribute to the pharmaceutical care of patients admitted for acute oncology reasons.
- Give specialist advice and support to medical and nursing staff, and other members of the pharmacy team.
- Give education and training to nurses and other members of the pharmacy team.
- Act as a pharmaceutical and prescribing adviser, including the production and review of medicines-related guidelines and pathways.
- Undertake clinical pharmacy practice at an advanced level.
- Provide professional supervision to technical and supporting staff and to undertake the role of Responsible Pharmacist when required.
- Contribute to the delivery and development of clinical pharmacy services across the Trust.
- Actively participate within the multi-disciplinary and clinical pharmacy team.
- Act as a source of expert advice on prescribing and medicines optimisation to medical and nursing staff within a defined clinical area.
- Participate in clinical audit and other initiatives aimed at assuring high quality prescribing and pharmacy practice.

Monday to Friday, 37.5 hours per week, 9am - 5pm, 30-minute break
