About the Role

We are seeking a highly motivated and enthusiastic Nurse Specialist to join our Intensive Support Team (IST) in Kirklees. As a key member of our team, you will provide specialist assessment, support, and treatment for individuals receiving intensive support and care, enabling service users to engage in a wide range of individual and group-based treatment activities.

Key Responsibilities
  • Provide specialist assessment, support, and treatment for individuals receiving intensive support and care.
  • Work closely with the IST multidisciplinary team to provide an intensive, wrap-around service to help improve the outcomes for service users.
  • Use your nursing skills to support the assessment and delivery of high-quality therapeutic interventions to address the holistic needs of service users.
  • Assess and review reports to feed into Positive Behavioral Support plans/interventions, liaison with families and support staff to implement communication interventions, and participate in meetings to help facilitate the coordination of cases.
  • Work proactively alongside ward staff, community agencies, and voluntary agencies to provide a seamless service for service users.
  • Maintain an effective communication network within the team and liaise with other agencies and disciplines from the voluntary and statutory sectors to ensure that service user needs are met.
  • Maintain up-to-date service user records and activity data in accordance with professional and Trust standards.
  • Provide and contribute to multi-disciplinary team meetings, ward rounds, clinics, and case conferences as required.
  • Empower families and carers to support individuals with learning disabilities.
  • Promote the positive health and well-being of the individual.
Requirements
  • Specialist knowledge and application of assessments and interventions relevant to learning disabilities.
  • Programme Approach, Risk assessment, and management.
  • Ability to reflect and critically appraise own performance.
  • Computer literacy.
  • Substantial post-registration experience working in learning disabilities.
  • Supervision of staff and students.
  • Experience of a range of mental health/learning disabilities settings, e.g., mental health positive behavior support, Autism.
  • RNLD or BSc/BA, MA Occupational Therapy.
  • Additional post-graduate qualification in positive behavior support at Diploma/Degree level.
  • HCPC State Registration/NMC registration.
