Norfolk County Council is dedicated to empowering young individuals with learning difficulties and/or disabilities, including mental health challenges and autism, to lead fulfilling lives. Our vision is to ensure these young people have equal access to opportunities in employment, housing, community engagement, and health services. To support this mission, we have established the Preparing for Adult Life Service, which facilitates a smooth transition into adult services for young people and their families.
We are seeking a motivated Practice Consultant with extensive experience in social work, particularly in handling complex needs and risks related to mental health or disabilities. The ideal candidate will possess a professional social work qualification and demonstrate a solid understanding of strength-based assessments, care management protocols, and supervisory skills.
This role requires the ability to engage with young individuals aged 14 to 21 years and their families, alongside a thorough comprehension of legislative frameworks, particularly the Care Act 2014 and the Mental Capacity Act. You will play a crucial role in guiding a team to fulfill these legal responsibilities.
A commitment to collaborative work with families and various professionals is essential to ensure comprehensive support for young people as they prepare for adult life.
