Experienced Physiotherapist for Community Care
3 months ago
Position Overview
An exceptional opportunity has emerged for a skilled Physiotherapist to become part of the North Cumbria Integrated Care NHS Foundation Trust. This role focuses on admission prevention, supported discharge, and specialized assessment and rehabilitation for patients facing a variety of long-term and short-term health challenges. Collaboration with health and social care professionals is essential to provide comprehensive care and support.
The ideal candidate will exhibit motivation and possess outstanding assessment and communication abilities, along with a strong aptitude for teamwork.
The service operates seven days a week, with flexible working hours between 8 AM and 8 PM. We encourage applications for part-time positions.
Key Responsibilities
The North Cumbria Integrated Care NHS Foundation Trust prides itself on exemplary multidisciplinary collaboration, delivering rehabilitation and rapid response care. You will primarily engage with elderly patients, individuals at risk of falls, and those managing chronic conditions.
Essential skills include effective communication, the ability to thrive in challenging settings, and a commitment to independent practice, providing evidence-based assessment, treatment, and rehabilitation.
This role offers opportunities to enhance clinical, supervisory, and managerial competencies. We are dedicated to the education of physiotherapy students from local universities, and you should demonstrate a commitment to your own professional growth as well as that of others.
We maintain strong partnerships with various stakeholders, including Commissioners, GPs, Acute Services, and community organizations. A primary objective is to deliver care within patients' homes to avert hospital admissions, facilitate timely discharges, and empower patients to manage their conditions, thereby improving their independence and quality of life.
Patients may be treated in community hospital settings or their own residences, including care homes, with the primary base being designated facilities.
