This is a challenging and rewarding opportunity to join our team as a Senior Occupational Therapist. As a key member of our Community Adult Mental Health team, you will be responsible for providing high-quality therapeutic interventions, education, and care coordination to mental health consumers, their carers, and family members.
Key Responsibilities- Provide case management services to allocated caseloads, including mental health assessments, risk assessments, and care coordination with other services.
- Develop and implement individualized treatment plans, incorporating a recovery-oriented framework.
- Collaborate with multidisciplinary teams to ensure holistic health care and promote optimal mental health outcomes.
- Contribute to quality improvement activities, including the development and review of performance indicators and policy development.
- Coordinate and administer the Sunflower Villas and Community Options accommodation program.
