About the Role

We are seeking a highly skilled and motivated Physiotherapist to join our Urgent Community Response team at the Royal Devon and Exeter NHS Foundation Trust. As a Specialist Physiotherapist, you will play a vital role in supporting patients in the community, working closely with multidisciplinary teams to ensure timely and effective care.

Key Responsibilities

  • Provide high-quality physiotherapy services to patients in the community, focusing on admission avoidance and hospital discharge.
  • Work collaboratively with nurses, occupational therapists, and other healthcare professionals to develop and implement individualized care plans.
  • Conduct thorough assessments and develop treatment plans to address patients' physical and functional needs.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in multidisciplinary meetings and contribute to the development of care pathways and protocols.

Requirements

  • Registered Physiotherapist with a full driving license.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
  • Commitment to delivering high-quality patient care and contributing to a culture of excellence.
