About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Island Health Primary Care LTD. As a Clinical Pharmacist, you will play a key role in providing expert medicines advice and support to our general practice staff, patients, and other healthcare professionals.

Key Responsibilities
  • Provide patient-facing long-term condition clinics, including COPD and asthma management
  • Deliver pharmacy-related aspects of the Network DES/LES, such as conducting structured medication reviews
  • Review ongoing medicine needs, monitoring requirements, and support patients with their medicines taking
  • Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacists, nurses, and/or GPs
  • Signpost patients to community pharmacy and refer to GPs or other healthcare professionals as needed
  • Provide telephonic medicines support and answer relevant medicine-related enquiries from GPs, practice staff, and patients
  • Manage medicines at discharge from hospital and assist with the development of a repeat prescribing process

Requirements
  • Masters degree in pharmacy (MPharm) or equivalent
  • Completed 18-month CPPE pathway or equivalent
  • Independent prescriber or actively working towards
  • Prior GP/clinical pharmacy experience and specialist in-depth knowledge for multiple chronic disease areas
