The Role: Registered Nurse - Community Care
We are seeking a dedicated and compassionate Registered Nurse to join our dynamic community nursing team. In this role, you will provide essential care and support to patients in their homes, promoting their independence and well-being.
Key Responsibilities:
- Delivering personalized nursing care to patients in their homes, based on individual needs and assessments.
- Developing and implementing care plans in collaboration with patients, families, and other healthcare professionals.
- Providing education and support to patients and their families on managing their health conditions.
- Maintaining accurate patient records and documentation.
- Working effectively as part of a multidisciplinary team to ensure seamless patient care.
Essential Criteria:
- Registered on Part 1 of the NMC register.
- At least one year of relevant experience post-qualifying as a Registered Nurse.
- Demonstrated commitment to upholding Trust Values.
- Recent experience working as a registered nurse in a community nursing role.
Desirable Criteria:
- Experience of CPD at Level 7.
