About the Role

We are seeking an experienced and motivated Community Health Nurse to join our team at Bridgewater Community Healthcare NHS Foundation Trust. As a Community Health Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their own homes or community settings.

Key Responsibilities
  • Conduct comprehensive assessments of patients' nursing needs and develop individualized care plans.
  • Provide evidence-based nursing care, including wound management, end-of-life care, and diabetic management.
  • Liaise with other healthcare professionals and agencies to ensure effective service delivery.
  • Support patients and their families in promoting health and wellbeing.
  • Participate in the coordination of complex and continuing care packages.
Person Specification
  • Registered Adult Nurse with NMC registration.
  • Previous community experience, including venepuncture and immunisation.
  • Able to assess, plan, implement, and evaluate packages of care.
  • Effective interpersonal skills and ability to develop relationships across the health and social care setting.
  • IV therapy experience.
  • Experience of working within palliative care.
