Job Summary:
We are seeking a highly skilled and experienced Practice Nurse to join our team at NHS Scotland. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the delivery of our healthcare services.
Key Responsibilities:
- Clinical Duties:
- Manage patients with chronic diseases, including diabetes, hypertension, and asthma, through regular clinic sessions and robust call and recall systems.
- Participate in national screening programs, such as cervical cytology, to promote early detection and prevention of diseases.
- Monitor and manage chronic diseases based on established protocols, including prescribing changes as necessary.
- Select, perform, and interpret results of specific tests to inform patient care.
- Provide direct and assist GPs with contraceptive services, including counseling and education.
- Act as a chaperone when required, ensuring patient comfort and dignity.
- Undertake health promotion activities to educate patients on healthy lifestyle choices and disease prevention.
- Supervise and support Healthcare Assistant (HCA) posts to ensure seamless patient care.
- Conduct clinics to interpret blood tests and administer medications as prescribed.
- Participate in Multidisciplinary Team (MDT) meetings to discuss patient care and treatment plans.
- Administrative Duties:
- Maintain accurate and up-to-date patient records, using agreed Read Codes to ensure consistency and clarity.
- Ensure accurate completion of all necessary documentation associated with patient care, including medical records and reports.
- Collect and analyze data to contribute to Practice audit and research projects, informing quality improvement initiatives.
- Contribute to the development of Practice development plans, aligning with NHS Scotland's strategic objectives.
- General Responsibilities:
- Maintain own annual registration with the Nursing and Midwifery Council (NMC) and adhere to the Code of Professional Conduct.
- Ensure a safe and therapeutic environment for all staff, patients, carers, and co-workers from other agencies, adhering to statutory regulations and guidelines.
- Promote a professional approach at all times, demonstrating a high level of attire and attitude towards patients and staff.
- Maintain own continuous professional development, ensuring Professional Development Plan (PDP) is up-to-date and aligned with Practice requirements.
- Attend and participate in staff and Practice meetings, as well as in-house clinical training sessions, to stay informed and up-to-date on best practices.
