Care CoordinatorWe are seeking a compassionate and dedicated Care Coordinator to join our growing team in Enfield. The ideal candidate will possess strong organizational skills, excellent communication abilities, and a genuine passion for helping people.
Responsibilities- Coordinate care plans for patients within the Primary Care Network (PCN), ensuring seamless transitions between healthcare providers.
- Liaise with GPs, nurses, social workers, and other healthcare professionals to facilitate effective communication and coordinated care.
- Provide administrative support to the PCN, including scheduling appointments, managing patient records, and handling correspondence.
- Support patients in accessing appropriate community resources and services.
- Contribute to the development and implementation of care pathways and protocols within the PCN.
- Proven experience in Primary Care, Community Care, Social Work, or Public Health with demonstrable experience in coordinating teams.
- Exceptional organizational skills and the ability to manage multiple tasks effectively.
- Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
- A compassionate and empathetic approach to patient care, with a commitment to providing high-quality service.
- Flexibility and adaptability to meet the evolving needs of the PCN.
- Strong teamwork skills and the ability to collaborate effectively within a multidisciplinary setting.
- Non-judgmental and non-discriminatory approach, upholding patient confidentiality at all times.
- Proficiency in IT skills, including MS Office Suite, Outlook email, and database software.
- Availability to work some evenings and weekends in community settings, demonstrating flexibility to meet service requirements.
- Experience supervising and developing volunteers, providing effective leadership, managing conflict, and utilizing negotiation skills when necessary.
- Knowledge of the local area and its challenges, with an understanding of the impact of disadvantage and social exclusion. A commitment to promoting inclusion and equality of opportunity for all.
- Own transport.
- Turkish, Somali, or Bulgarian language skills would be advantageous.
