We are seeking a highly motivated and experienced Cardiac Rehabilitation Specialist Nurse to join our team at Gloucestershire Health and Care NHS Foundation Trust.
Key Responsibilities- Conduct comprehensive nursing and holistic assessments for patients with Coronary Heart Disease (CHD), analyzing complex information to inform appropriate care and management.
- Develop and implement personalized care plans, incorporating medication and lifestyle advice to promote optimal health outcomes.
- Collaborate with multidisciplinary teams to provide effective care and support, including liaison with primary and secondary healthcare professionals.
- Act as a clinical expert, providing education and training to colleagues and patients on cardiovascular disease management and prevention.
- Contribute to the development and improvement of cardiac rehabilitation services, including audit and quality improvement initiatives.
- Registered Nurse with the Nursing and Midwifery Council (NMC)
- Degree or diploma in Nursing
- Experience working with patients with long-term conditions, including coronary heart disease
- Ability to work flexibly, including evenings and weekends, to deliver high-quality care
- Current valid driving license and access to a car
