Care Manager

Active Care Group is a leading provider of high-quality care services, and we are seeking an experienced Care Manager to join our team. As a Care Manager, you will be responsible for delivering exceptional care to our clients in their own homes, ensuring their safety, reliability, and well-being.
Key Responsibilities:- Develop and implement care plans that meet the unique needs of each client
- Build strong relationships with clients, families, and external partners to ensure seamless care delivery
- Lead a team of support workers to provide high-quality care services
- Monitor and evaluate care services to ensure they meet our high standards
- Collaborate with our clinical and business leadership teams to drive continuous improvement
- NVQ 4 or above in Health and Social Care
- Full clean driving license with access to own car
- Experience in care at Team Leader level or above
- Passion for delivering high-quality client-focused care
- Positive attitude and commitment to continuous improvement and quality
- Salary up to £30,000 per annum pro rata
- 25 days annual leave, plus your birthday off
