Support Worker Opportunity

We are seeking a dedicated and compassionate Support Worker to join our team at Active Care Group. As a Support Worker, you will play a vital role in providing high-quality care to our residents with acquired brain injuries or long-term neurological conditions.

Key Responsibilities:
  • Provide individualised care to residents, promoting their independence and well-being.
  • Work collaboratively with the clinical team to support residents in achieving their planned outcomes.
  • Build strong relationships with residents, relatives, and carers, ensuring open and effective communication.
  • Support residents in their rehabilitation programme, promoting their physical and emotional well-being.
  • Establish methods of communication to reduce barriers and ensure residents' needs are met.
  • Assist with activities of daily living and personal care, ensuring residents' dignity and comfort.
  • Communicate any issues or concerns to the clinical team, ensuring prompt action is taken.
  • Accurately record and report work activities, maintaining high standards of documentation.
