This role is an integral part of a collaborative health and social care framework, bringing together various professionals including Social Workers, Community Nurses, Psychologists, Occupational Therapists, Speech and Language Therapists, Art Therapists, Physiotherapists, and Psychiatrists.

The Support Services Coordinator will be a key member of the social work team.

Key Responsibilities:

The position involves direct engagement with service users, their families, and external stakeholders.

The primary objective of the Support Services Coordinator is to assess whether existing support packages effectively address the needs of service users and their families.

Successful candidates will be responsible for making informed recommendations regarding support packages and facilitating adjustments to ensure that needs are met in a personalized and strengths-based manner.

Essential Qualifications:

Strong verbal and written communication skills are essential for this role.

The Support Services Coordinator will participate in multidisciplinary meetings, present cases, and communicate effectively with service users and their families, as well as a diverse range of internal and external colleagues.

Additionally, the Coordinator will be tasked with drafting precise and comprehensive support plans and assessments.
