About the Role

We are seeking a highly skilled and dedicated Registered Nurse to join our Community Staff Nurse team at Powys Teaching Health Board. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality nursing care to patients in their own environment.

Key Responsibilities
  • Provide holistic assessment and care to patients with complex and palliative needs
  • Develop and implement evidence-based care plans to promote patient independence and well-being
  • Work collaboratively with the District Nursing Team to deliver efficient and effective care services
  • Prioritize delegated home visits based on patient clinical need and geographical location
Requirements
  • NMC Registered Nurse qualification
  • Excellent interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Good IT skills and ability to travel between sites
Desirable Qualifications and Skills
  • Competence in palliative care, chronic disease management, and wound management
  • Ability to speak Welsh
  • Working towards diploma/degree/masters qualification in CHS (District Nursing) Specialist Practitioner DN
