About the Role

We are seeking a skilled Mental Health Practitioner to join our Home Crisis Treatment team in South West Staffordshire. As a key member of our team, you will work closely with Staffordshire Police and Mental Health Services to provide safe and effective home treatment for individuals experiencing mental health crises.

Main Responsibilities
  • Assess and support individuals referred to the service, developing and implementing personalized wellbeing and recovery action plans.
  • Deliver high-quality nursing care to service users, working collaboratively with multidisciplinary teams to achieve optimal outcomes.
  • Monitor and evaluate the effectiveness of care, implementing improvements as needed.
