Job Summary

We are seeking a dedicated and compassionate Support Worker to join our team at Disability North. As a Support Worker, you will play a vital role in supporting an individual with complex needs, enabling them to live a fulfilling and independent life.

Key Responsibilities

  • Support an individual with daily living skills, personal care, and independent life skills
  • Assist with accessing community-based activities, such as swimming, music, and social events
  • Develop and implement individualized care plans to meet the needs of the individual
  • Work collaboratively with the care team to ensure seamless support and care
  • Maintain accurate records and reports of the individual's progress and well-being

Requirements

  • Experience working with adults with Learning Disabilities, Autism, or complex behavioral needs
  • Enhanced DBS check required
  • Ability to work a shift rota basis, including evenings and weekends
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
