About the Role

The successful candidate will be responsible for providing high-quality support and care to our service users, ensuring their physical, emotional, and social needs are met. This will involve working closely with our team to develop and implement personalized support plans, promoting independence and empowering individuals to reach their full potential.

Key Responsibilities

  • Develop and maintain effective relationships with service users, their families, and other stakeholders
  • Conduct thorough assessments to identify individual needs and preferences
  • Collaborate with the team to create and implement support plans that promote independence and well-being
  • Provide emotional support and guidance to service users, promoting confidence and self-esteem
  • Monitor and evaluate the effectiveness of support plans, making adjustments as needed
